Job summary
The Hampton-Newport News Community Services Board (H-NNCSB) is seeking a compassionate Residential Nurse (LPN) to support individuals with developmental disabilities in ICFs and Waiver Homes. You will provide hands-on nursing care, coordinate medical needs, and work with families and staff to promote health, independence, and safety.
The role requires an accredited LPN program, Virginia license, and at least one year of clinical experience, with on-call duties and a sign-on bonus.
Qualifications
- Graduation from an accredited LPN program and valid LPN license in Virginia.
- Minimum 1 year of clinical experience including medication administration in community, hospital, or nursing home settings.
- Experience with IDD/REACH and/or mental health is preferred.
Responsibilities
- Conduct nursing assessments, develop and implement care plans, monitor health status and respond to concerns.
- Coordinate medical appointments, referrals, transportation, provider communication and follow-up.
- Oversee medication management including storage, administration, documentation and disposal.
- Provide guidance and training to residential staff and families during interdisciplinary planning.
