Job summary

Signature HealthCARE of Norfolk is seeking an experienced LPN/LVN to provide direct nursing care in a 169-bed facility in Virginia. You will administer medications, assist with ADLs, and supervise nursing aides on your shift, while ensuring comfort, dignity, and safety for residents.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role is preferred.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Strong observation, medication administration and documentation skills.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify attending physician of any changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
