Job summary

Four County Health and Rehabilitation in Georgia is seeking a part-time nursing professional to support care planning, assessments and interdisciplinary collaboration in a non-profit setting.

You will facilitate communication between nursing center departments, patients and families, participate in assessment interviews, help develop and refine care plans, and ensure timely documentation in accordance with regulations while upholding compassionate care.

Qualifications

• Nursing Degree from an accredited college or university or is a graduate of an approved LPN program.
• An active LPN License to practice in the State of Georgia.
• Proficient in using computer software and standard office applications.

Responsibilities

• Facilitates communication between nursing center departments and patient/family members for effective assessment/care planning.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Assists Director with patient/family interviews and evaluation of patient conditions.
• Helps develop preliminary and comprehensive care plans with measurable goals.
• Ensures assessments and care plans are completed timely and in accordance with regulations.
