Job summary

Pioneer Ridge Assisted Living is seeking a Licensed Practical Nurse to provide direct nursing care to residents in a long-term care setting. You will follow federal, state, and local standards, and collaborate with the RCC Services or Nurse Supervisor to ensure high-quality care.

Responsibilities include facilitating admissions and discharges, assisting with daily hygiene, documenting care, and reporting any changes to the Charge Nurse.

Qualifications

• Must possess a valid LPN license in good standing with the state.
• Strong ability to document care and communicate with the team.
• Experience in long-term care settings preferred.

Responsibilities

• Provide direct nursing care to residents in accordance with regulations.
• Maintain confidentiality of resident information and comply with policies.
• Document care and report changes to the Charge Nurse/Nurse Supervisor.
• Assist in admission, discharge, and daily hygiene care.
• Assist with mobility, transfers, and transportation within the facility.
• Conduct routine nursing procedures and charting as required.
