Job summary

Greater Good Health is seeking a Licensed Practical Nurse to provide holistic, high-touch care for a panel of senior patients at the GGH Clinic in Chubbuck, ID. You will collaborate closely with Nurse Practitioners to deliver value-based primary care in a supportive, on-site clinic environment.

You will perform vital signs, screenings, and coordination tasks to ensure timely visits and excellent patient experience, while supporting lab activities and ensuring safety standards are met.

Qualifications

• Graduate of an accredited Licensed Practical Nursing program.
• Valid LPN license.
• 3+ years of experience as an LPN; primary care experience a plus.
• 1+ year of Phlebotomy experience preferred.
• Current BLS certification endorsed by the American Heart Association preferred.

Responsibilities

• Deliver outstanding clinical care and support functions related to patient care activities.
• Actively engage and manage patient panels with the Nurse Practitioner.
• Ensure efficient clinic flow by rooming patients promptly and assisting the provider.
• Conduct visit activities: taking vitals, screenings, histories, preparing patients for examination.
• Support post-visit activities: referral completion, patient care coordination, and follow-up outreach.
