Job summary

Elara Caring in Lawrence, MA is seeking a Licensed Practical Nurse (LPN) for full-time, in-home care. You will deliver nursing services per Plan of Care, review plans before visits, and document patient updates while traveling to homes roughly 50% of the time.

Requirements include an MA LPN license, graduation from an accredited nursing program, at least 1 year of acute care experience (home health or hospice preferred), strong communication and time-management skills, and reliable

• Current LPN/LVN license in MA.
• Graduate of an accredited nursing program.
• At least 1 year of experience in acute care (home care, home health or hospice preferred).
• Strong communication and interpersonal skills.
• Ability to manage time effectively and stay organized in the field.
• Knowledge of healthcare regulations and patient care standards.
• Reliable transportation (travel required ~50%).
• Ability to meet health screening and compliance requirements.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow Plan of Care prior to each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and care team.
