Job summary

CenterWell Home Health is seeking a dedicated Licensed Nurse (RN/LVN) to coordinate patient visits and maintain care coordination with field staff, physicians, and patients. This role emphasizes scheduling, infection control processes, and collaboration across branches to ensure continuity of care.

The position requires home health experience, a valid driver’s license, and a current state nursing license. It also involves occasional on-site patient visits and on-call rotation as needed.

Qualifications

• Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which he / she practices.
• Have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.
• Must possess a valid state driver’s license and automobile liability insurance.
• Must be currently licensed in the State of employment if applicable.
• Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.

Responsibilities

• Manages schedules for all patients and edits schedules for agents calling in sick to reassign patients timely.
• Initiates infection control forms and uploads the Employee Infection Report to the worker console.
• Serves as back up during lunch and busy times, answers field staff calls, and assists with weekly case conferences.
• Maintains the client hospitalization log and shares coordination notes with office, field, and sales staff.
• Completes requested schedules for all add-ons and applicable orders and ensures staff are scheduled for skilled nurse/injection visits as needed.
• Schedules discharge visits / OASIS collection or recert visits following case conferences.
• Reschedules declined or missed visits and processes reassigned visits.
