Job summary

ABLE, INC. in Dickinson, ND is seeking a Licensed Practical Nurse to coordinate medical care for individuals served, work with a multi-disciplinary team, and ensure high-quality health outcomes.

The role requires collaborative communication with guardians, families, physicians and care staff to implement personalized health plans. The LPN will monitor health status, schedule appointments, provide health education, and support staff in nursing tasks, while maintaining compliance with ICF/IDD

Qualifications

• Currently licensed as a Licensed Practical Nurse in North Dakota.
• Prefer 1 year of experience working with people with developmental disabilities.
• Maintain CPR and First Aid certification.
• Valid driver’s license.
• Basic computer knowledge.
• Experience with ICF/IDD and The Council standards is preferred.

Responsibilities

• Respond to accidents, injuries, and illnesses and intervene as needed.
• Schedule follow-up appointments and document progress.
• Monitor health care progress and update medical records.
• Coordinate appointments, labs, imaging, and therapies; arrange transportation when needed.
• Ensure appropriate post-visit follow-up and communications with staff, family, and coordinators.
• Coordinate hospital admission and discharge and out-of-town travel for appointments.
• Provide health care education to people supported and staff as needed.
• Assess suspected abuse/neglect and communicate with health services coordinator.
• Review referral packets to determine ABLE, Inc.'s ability to support health needs.
• With health services coordinator, train staff for nursing tasks and ensure competency.
• Assist in quarterly physicals and med administration recertification.
• Communicate health issues to health services coordinator and executive director.
• Serve as liaison with medical community and promote ABLE, Inc. services.
• Cover health care needs when the health services coordinator or other nurses are unavailable.
• Participate in agency committee meetings and comply with standards and ABLE policies.
• Complete and submit health updates, incident reports, timesheets, expense reports, and logs.
