Qualifications

• Licensed Practical Nurse licensure required (LPN).
• Active LPN license with state board requirements or compact licensure accepted.
• Valid driver's license with acceptable driving record.
• BLS certification required or within 90 days of start date.

Responsibilities

• Assist in patient assessments and document nursing interventions.
• Support discharge planning and patient education.
• Collaborate with the care team for continuous patient-centered care.
• Delegate nursing tasks to qualified staff as appropriate.
• Identify early signs of patient condition changes and respond promptly.
• Communicate patient information with families and team members.
• Document pain reports and coordinate with continuous care plans.
• Participate in health education approved by a licensed practitioner.
• Contribute to safety and quality initiatives within the department.
