HIA Primary Care in Fargo, ND is seeking a Licensed Practical Nurse (LPN) to provide nursing care in coordination with the interdisciplinary team. You will serve as a care coordinator for assigned providers, handling intake, assessments, care coordination, scheduling and other medical practice duties.

The role emphasizes collaboration with high-risk patients, promotes preventive care, and supports patient education and self-management within regulatory guidelines and EMR documentation standards.

Qualifications

• Minimum of LPN degree.
• Minimum of 3 years nursing experience in clinical setting.
• Current LPN licensure in North Dakota and Minnesota (or willing to obtain dual licensure).

Responsibilities

• Provide nursing care in coordination with the interdisciplinary team for Primary Care patients.
• Assess patient needs, perform intake, and assist with care coordination and scheduling.
• Enter and follow up with orders from providers in the EMR, ensuring accurate documentation.
• Monitor high risk patients, promote preventive care, and educate patients for self-management.
