Job summary

Jobtailor is seeking a dedicated LVN/LPN to deliver skilled nursing care in patients’ homes. You will implement individualized plans, monitor changes, educate families, and document progress for seamless care coordination with the RN Case Manager.

The role emphasizes compassionate, patient-centered care, safety, and collaboration with physicians and care teams. Travel within the service area is required.

Qualifications

• Graduate of an accredited Associate Degree Nursing program.
• Current state LVN/LPN license.
• Minimum of one year of clinical nursing experience.
• Home Health or Hospice experience preferred, not required.
• Strong communication, critical thinking and patient care skills.
• Ability to lift, push, pull and support up to 50 pounds.
• Valid driver’s license or reliable transportation to travel to patient homes.
• Willingness to travel within the assigned service area.

Requirements

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Willingness to travel within the assigned service area
