- Deliver skilled nursing care under the direction of a Registered Nurse and physician
- Follow and implement individualized Plans of Care
- Monitor patient conditions and communicate changes to the RN Case Manager and care team
- Educate patients and caregivers on disease management, medications, and self-care techniques
- Document assessments, interventions, and patient progress accurately and timely
- Promote patient safety and implement strategies to reduce avoidable hospitalizations and emergency room visits
- Collaborate with physicians, nurses, and interdisciplinary team members to ensure quality patient outcomes
- Support patients and families with compassionate, respectful, and patient-centered care
- Identify opportunities for improvement and communicate recommendations to the RN Case Manager
Requirements
- Graduate of an accredited Associate Degree Nursing program
- Current state LVN/LPN license
- Minimum of one (1) year of clinical nursing experience required
- Home Health or Hospice experience preferred not required
- Strong communication, critical thinking, and patient care skills
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
- Willingness to travel within the assigned service area
