Job summary

A community health organization in Youngstown, Ohio is seeking a caring and reliable LPN for a part-time per diem role. The successful candidate will provide quality care to tenants in an assisted living environment, assist with assessments, monitor tenants' conditions, and support the team during shifts. Duties include but are not limited to:

• Provide quality tenant care in accordance with assisted living philosophy.
• Assist with tenant assessments, screenings, medication assistance and administration, and care plan updates as assigned.
• Monitor and document tenants’ physical and mental condition and report significant changes to appropriate parties.
• Perform evaluations as needed based on tenant condition or assignment.
• Demonstrate appreciation of Jewish traditions and understanding of the aging process.
• Provide support to the Director and team as needed during scheduled or on-call shifts.
• Maintain confidentiality and adhere to HIPAA and organizational policies.

Requirements
