Job summary

Ovation Communities - Jewish Home & Care Center in Milwaukee is seeking a dedicated LPN to provide general nursing care to residents on the AM shift. You will administer medications, perform treatments, and help assess and support residents’ spiritual, physical, emotional, and social needs in accordance with the care plan.

You will work with CNAs, document care, update plans as conditions change, and ensure accurate charts in compliance with policies and regulations.

Qualifications

• LPN licensure required.

Responsibilities

• Receives verbal report and communicates care information to CNAs.
• Visits residents to ensure appropriate cares are provided; reports problems to supervisor.
• Administers and documents medications and treatments per plan and licensure.
• Reports significant changes in residents' conditions to supervisor.

Ovation Communities is seeking a LPN to provide general nursing care, including treatments, administering of medication to residents, and preparing and assessing examinations. The LPN provides personal care services to nursing home residents to meet individuals spiritual, physical, emotional and social needs consistent with the plan of care, Standard Nursing practice, scope of gerontological nursing procedures and all applicable regulations, codes, policies and procedures

Key Responsibilities:

1. Receives verbal report and communicates all pertinent resident care information to the CNAs on assigned unit or area.
2. Visits each resident to ensure the appropriate cares are being provided, reports any problems to the manager/supervisor.
3. Administers and documents medication and treatments, etc. within the parameters of the LPN licensure and according to the care plan. Reports any changes to the manager/supervisor.
4. Reports significant changes in residents' conditions immediately to the manager/supervisor and notifies appropriate people as directed.
5. Maintains timely, accurate written charts, documents, records as required by law and/or regulation policies and procedures.
6. Review and revise care plans as changes occur to reflect the exact condition of each resident.
