Job summary

Ann's Choice by Erickson Senior Living is seeking a Licensed Practical Nurse (LPN) to care for residents in our skilled nursing neighborhood. The role covers the 11pm–7am shift, requiring compassion, accuracy in med administration, and EMR documentation.

Eligible nurses receive a comprehensive benefits package and opportunities for growth within a expanding senior living network. Minimum 1 year in long-term care or related setting, LPN/LVN license, and CPR certification are required.

Qualifications

• Minimum 1 year experience in assisted living, home health, or long-term care.
• Licensed LPN/LVN per state.
• CPR certification required.
• Experience with electronic medical records preferred.
• State regulations may apply.

Responsibilities

• Developing relationships with residents and families with a consistent caregiver approach to support physical, spiritual, emotional, and clinical needs.
• Assessing, observing, and documenting residents’ clinical condition in the EMR.
• Updating or modifying care plans as needs change and reflecting it in documentation.
• Supervising and leading care associates; administering medications timely and accurately.
• Participating in meetings related to resident outcomes and responding professionally to questions.

Job description

Location: Ann's Choice by Erickson Senior Living Join our team as a Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN). The LPN/LVN is responsible for providing clinical care to residents in the neighborhood according to the nursing standards of practice, board of nursing, and Erickson Senior Living standards. Schedule: 11p-7am, 1 weekday and every other weekend in Skilled. Compensation: up to $37/hr for LPN, and $49/hr for RN--- plus differential.

How you will make an impact

Developing relationships with the residents and their families using a consistent caregiver approach supporting the resident’s physical, spiritual, emotional, and clinical needs and focusing on placing the resident first. Assessing, observing, and evaluating resident’s clinical condition, document findings in the resident electronic medical record, and notify the provider and/or RN when necessary. Updating or modifying care plans as needed to reflect any changes to a resident’s care and service needs. Supervising and leading care associates Administering medications timely, accurately, and according to orders. Participating in meetings related to resident outcomes and care Responding to residents, family, or responsible parties in a timely and professional manner Participating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc. Other duties as assigned by RN clinical leader

What you will need

Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred. Licensed by State as a Licensed Practical Nurse. CPR certification required. A passion to care for seniors and be a strong resident advocate. Experience with electronic medical records is preferred.
