Signature HealthCARE is seeking a caring LPN/LVN to provide direct nursing care and supervise nursing staff in a facility setting. You will administer medications, assist with ADLs, and ensure resident safety and comfort.

The role requires an active LPN/LVN license in the state, CPR/BLS certification is preferred, and at least one year of clinical nursing experience, with long-term care experience preferred. A supportive team and growth opportunities are offered.

Competenze

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.

Mansioni

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
