Monarch Healthcare Management is seeking a licensed practical nurse (LPN) in Minnesota to provide direct resident care under nursing leadership. The role includes participating in Plan of Care development and ensuring interventions are carried out as prescribed, while monitoring resident health data and reporting changes to the RN or physician.

The LPN will supervise unlicensed nursing staff as needed and support residents’ physical, mental, and psychosocial well-being.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse.
• Current Certification in CPR.

Responsibilities

• The LPN provides residents with direct routine care under nursing leadership.
• Develops and modifies the Plan of Care and implements delegated interventions.
• Collects resident health data and reports changes to the RN.
• Supervises tasks of unlicensed nursing staff and clarifies expectations.
