Job summary

Monarch Healthcare Management is seeking a Licensed Practical Nurse to provide direct routine care under nursing leadership in Minnesota. The LPN participates in developing and modifying the Plan of Care, implements delegated interventions, collects health data, and reports changes to the RN.

This role supports residents’ physical and psychosocial well-being and requires a Minnesota LPN license and CPR certification.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse.
• Current Certification in CPR.

Responsibilities

• Provide residents with direct routine care under nursing leadership.
• Develop and modify the Plan of Care; implement delegated interventions.
• Collect resident health data and report changes to the RN.
• Assign and monitor tasks to unlicensed nursing staff after assessing their capabilities.
• Support residents in achieving highest practicable physical, mental, and psychosocial well-being.
