Job summary

Monarch Healthcare Management is seeking a Licensed Practical Nurse (LPN) to deliver high-quality direct routine care under nursing leadership in Minnesota. You will help develop and modify the Plan of Care and report changes to the RN or physician.

The LPN will assign and monitor tasks for unlicensed nursing staff, ensuring residents receive consistent, safe care while supporting their physical, mental, and psychosocial well-being.

Qualifications

• Current Minnesota Licensed Practical Nurse license.
• Active CPR certification as required.
• Experience in long-term care or skilled nursing desirable.

Responsibilities

• Provide residents with direct routine nursing care under supervision.
• Participate in developing and modifying the Plan of Care with the RN or physician.
• Collect health data and report changes or responses to interventions promptly.
• Assign and monitor tasks for unlicensed nursing staff and ensure safety.
