Job summary

Beloit Health System is seeking Per Diem Med Pass LPNs for the NorthPointe Terrace team in Illinois. LPNs will administer medications to residents needing assistance with nursing and personal care, with duties aligned to the hospital and NorthPointe Terrace values.

Responsibilities include assessing condition changes, transcribing new orders, reviewing labs and meds, and supporting PCAs during passes. Flexible hours and limited benefits accompany this per diem role.

Qualifications

• Must be licensed as an LPN in Illinois.
• Ability to think, act, and intervene independently in routine and emergency situations.
• Self-motivated and resident centered.
• Able to relate to all residents and staff in a courteous, professional, and compassionate manner.
• Able to work flexible hours and participate in holiday activities with residents.
• CPR certification within 90 days of hire.
• Maintains confidentiality at all times.
• Works collaboratively with peers and team members.
• Follow up promptly and thoroughly with appropriate staff, residents, or other individuals regarding complaints, problems, and concerns.
• Adheres to established hospital and NorthPointe Terrace policies, rules, and regulations.

• Reviews labs, orders and medications with monitoring of administration and completion of documentation in relation to each review
• Participates as a resource and team member for the PCAs during medication passes.
• Used base of knowledge to educate staff, residents and families
• Writes care notes on any changes in resident condition and follows up on previous notes

The LPN– Med Pass administers medications to residents who are functionally, physically, or socially impaired and who need assistance with supportive, nursing, and personal care. All responsibilities will be conducted in a manner that is consistent with the philosophy of Beloit Memorial Hospital's and NorthPointe Terrace's mission, vision, and values.
