Provide direct patient care to assigned residents, including medication administration, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Maintain accurate and detailed records of patient care, treatment, and medication administration
Participate in quality improvement initiatives to enhance the overall quality of care
Collaborate with other healthcare professionals to ensure seamless transitions and effective communication
Maintain a clean and safe environment, adhering to established infection control and safety protocols
Current LPN licensure in the state of Wisconsin
High school diploma or equivalent required
Previous experience in a long-term care setting preferred
Possess excellent communication and interpersonal skills
Ability to work a variable shift schedule, including evenings and weekends
Reliable transportation and a valid driver's license (if applicable)
