Job summary

St. Francis Hospice in Melbourne, FL is seeking a Licensed Practical Nurse for night shifts (7 pm–7 am) to join our compassionate care team. A $1,500 bonus is available.

The LPN will assess patients, collect data, and provide PRN visits and crisis care as needed, working with the interdisciplinary team to ensure comfort and quality of life. Florida LPN license is required; hospice, home health, or palliative care experience is preferred.

Qualifications

• Current and active Florida LPN license is required.
• Hospice, home health, palliative care, or related experience preferred.

Responsibilities

• Assess each patient’s status, problems and needs.
• Collect and analyze data from medical history, nursing assessments and staff input on an ongoing basis.
• Provide PRN visits on patients who call the answering service and crisis care visits as assigned.

St. Francis Hospice (SFH) is growing, and we need a LPN to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.

The Licensed Practical Nurse (LPN) is responsible for the accurate assessment of each patient’s status, problems and needs. The LPN collects and analyzes data from the patient’s medical history, nursing assessments and staff input on the patient’s medical history, nursing assessments and staff input on an on going basis. The LPN provides PRN visits on patients who call the answering service, crisis care visits as assigned and PRN visits as requested by PCC or MD. Provides care for patients for the entire County. The LPN works collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life.

Current and active Florida (LPN) License required. Hospice, home health, palliative care, or related experience preferred.
