Job summary

Treasure Coast Hospice admissions team seeks an experienced LPN to review newly admitted patient charts for accuracy, QA admission forms, and follow up with patients on the NTUC list, providing information about hospice services.

You will handle incoming calls from patients, caregivers and medical professionals, relay information accurately, and may visit patients when needed. This on-site role in Fort Pierce, FL offers a $1,500 sign-on bonus and flexible day shifts.

Qualifications

  • Current CPR certification.
  • Minimum of three years of nursing experience in a home health/hospice setting within the past five years is required.
  • Preferred knowledge of hospice, Medicare, Medicaid and private insurance benefits.
  • Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
  • Must be self-directed and able to work independently or as a team.
  • Basic computer skills, use of common office software and EMR software experience.
  • Excellent people and relationship-building skills. Excellent customer service skills.
  • Maintain effective work relationships with co-workers, the public and administration.
  • Highly organized, adaptable problem solver and ability to work under stress or pressure.
  • Attendance is a condition of employment. Ability to work a variety of shift hours, to include days, nights, weekends and holidays.

Salary Range: $25.00 To $26.00 Hourly

$1,500 Sign-on-Bonus! M - F 8am - 4:30pm

The LPN Admissions Care Coordinator primary role will be responsible for reviewing newly admitted hospice patient’s charts for accuracy and will QA all necessary forms needed for admissions. He/she will follow-up with patients on the NTUC list & provide additional information about hospice services.

Responsible for answering incoming telephone calls from patients/caregivers/medical professionals. Ability to react quickly and calmly, exercise good judgement and make sound decisions during urgent and routine communications. Proficient in receiving, obtaining and relaying accurate information to be disseminated. Provides skilled supportive patient and family care over the telephone in accordance to certification restrictions. May be required to visit patients when needed. Provides care within scope of practice.
