Job summary

St. Francis Hospice is seeking an LPN to join our team for night shifts from 7pm to 7am. The LPN will assess patient status, gather data from medical history and nursing assessments, and provide PRN visits as needed.

The role requires an active Florida LPN license and hospice or palliative care experience is preferred. We offer a comprehensive benefits package and a mission-driven environment focused on comfort, dignity, and quality of life for patients and families.

Qualifications

• Current and active Florida LPN license.
• Hospice, home health, or palliative care experience preferred.
• Ability to cover 7pm–7am shifts.

Responsibilities

• Assess patient status, problems, and needs.
• Collect/analyze data from history, nursing assessments and staff input.
• Provide PRN visits and crisis care visits as assigned/requested.
• Care for patients across the entire County.
• Collaborate with the interdisciplinary care team to ensure comfort and dignity.

St. Francis Hospice (SFH) is growing, and we need a LPN to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.

The Licensed Practical Nurse (LPN) is responsible for the accurate assessment of each patient’s status, problems and needs. The LPN collects and analyzes data from the patient’s medical history, nursing assessments and staff input on an on going basis. The LPN provides PRN visits on patients who call the answering service, crisis care visits as assigned and PRN visits as requested by PCC or MD. Provides care for patients for the entire County. The LPN works collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life.

Current and active Florida (LPN) License required. Hospice, home health, palliative care, or related experience preferred.
