Job summary

St. Francis Hospice is seeking a Licensed Practical Nurse (LPN) to join our caring team in the community. The LPN will perform patient assessments, collect and analyze data, and coordinate with the interdisciplinary care team to ensure comfort, dignity and quality of life for patients and families.

Florida-licensed LPN required; hospice, home health or palliative care experience preferred. We offer a comprehensive benefits package and growth opportunities.

Responsibilities

• Perform accurate patient status assessments and analyze data from medical history and nursing assessments.
• Provide PRN visits and crisis care as assigned, in collaboration with PCC or MD.
• Care for patients across the County and work with the interdisciplinary care team to ensure comfort and quality of life.

St. Francis Hospice (SFH) is growing, and we need a LPN to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.

The Licensed Practical Nurse (LPN) is responsible for the accurate assessment of each patient’s status, problems and needs. The LPN collects and analyzes data from the patient’s medical history, nursing assessments and staff input on the on going basis. The LPN provides PRN visits on patients who call the answering service, crisis care visits as assigned and PRN visits as requested by PCC or MD. Provides care for patients for the entire County. The LPN works collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life.

Current and active Florida (LPN) License required. Hospice, home health, palliative care, or related experience preferred.
