Job summary

St. Francis Hospice is seeking a Licensed Practical Nurse (LPN) to join our team in Florida. The LPN will assess patient status, collect data from medical histories, and collaborate with the interdisciplinary care team to ensure comfort and quality of life.

Active Florida LPN license is required; hospice, home health, or palliative care experience is preferred. This role offers a $1,500 bonus and a comprehensive benefits package with medical, dental, vision, disability, life insurance, PTO,

Qualifications

• Must hold an active Florida LPN license.
• Prior hospice, home health or palliative care experience preferred.
• Ability to assess patient status and collaborate with care team.

Responsibilities

• Assess patient status and problems based on history and nursing assessment.
• Collect and analyze patient data for ongoing care planning.
• Provide PRN visits and crisis care as needed.
• Collaborate with PCC/MD and care team to maintain comfort and quality of life.

St. Francis Hospice (SFH) is growing, and we need a LPN to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.

The Licensed Practical Nurse (LPN) is responsible for the accurate assessment of each patient’s status, problems and needs. The LPN collects and analyzes data from the patient’s medical history, nursing assessments and staff input on an on going basis. The LPN provides PRN visits on patients who call the answering service, crisis care visits as assigned and PRN visits as requested by PCC or MD. Provides care for patients for the entire County. The LPN works collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life.

Current and active Florida (LPN) License required. Hospice, home health, palliative care, or related experience preferred.
