Job summary

St. Francis Hospice in Melbourne, FL is seeking a Licensed Practical Nurse for night shifts (7 pm–7 am) to join our compassionate care team. A $1,500 bonus is available.

The LPN will assess patients, collect data, and provide PRN visits and crisis care as needed, working with the interdisciplinary team to ensure comfort and quality of life. Florida LPN license is required; hospice, home health, or palliative care experience is preferred.

Qualifications

• Current and active Florida LPN license is required.
• Hospice, home health, palliative care, or related experience preferred.

Responsibilities

• Assess each patient’s status, problems and needs.
• Collect and analyze data from medical history, nursing assessments and staff input on an ongoing basis.
• Provide PRN visits on patients who call the answering service and crisis care visits as assigned.
• Work with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity and quality of life.
