Job summary

Center for Human Development (CHD) seeks a full-time Licensed Practical Nurse (LPN) to lead medication administration oversight across CHD's congregate care network. You will coach staff, review records, and coordinate with prescribers, pharmacies, and providers to ensure safe and compliant care.

Qualifications

• Must hold a Licensed Practical Nurse (LPN) license.
• Experience in medication administration and clinical oversight preferred.
• Ability to coach staff and ensure compliance with policies.

Responsibilities

• Review medication administration practices for compliance with orders, licensing standards, and CHD policies.
• Verify medications are ordered, stored, documented, and administered correctly.
• Monitor vital signs, glucose, allergies, and prescriber notifications as needed.
• Observe staff administering medication and complete competency observations.
• Identify training needs and provide coaching and in-service education.
• Review MORs, identify patterns, and coordinate corrective actions.
• Support medication reorders, reviews, and prescriber paperwork with providers.
• Maintain medication rooms and report deficiencies to program leaders.
