Liberty Community Living Center is seeking a Medical Records Specialist - LPN for the day shift to manage long-term care documentation and comply with state and federal regulations. The role supports nursing staff and ensures accurate medical records across software and paper systems.

Ideal candidate holds an active LPN license with experience in medical records and PDPM/ICF processes. Strong attention to detail and communication are essential for audits and departmental coordination.

Qualifications

Active LPN license.
Knowledge of long-term care documentation and regulations.
Experience with medical records management.

Responsibilities

Provide daily reporting during clinical stand up including past due forms and orders.
Compile and set up residents' medical records and enter data into software system.
Collect information from nursing staff and physicians and update data in software and hard copy.
Monitor and audit records for completeness and accuracy per policies and regulations.
Provide resident information to approved personnel as needed.
Process transfer or discharge records and conduct discharge auditing.
