Qualifications

  • Current Oregon LPN license in good standing.
  • Minimum 2 years in nursing or medical records settings.
  • Strong attention to detail and organization.
  • Ability to manage multiple priorities and deadlines.
  • Excellent written and verbal communication skills.
  • Knowledge of HIPAA and healthcare regulations.
  • Familiarity with ICD-10-CM coding and indexing.
  • Experience with EMR/EHR systems.
  • RHIT credential preferred.
  • Collaborative, dependable team player.

Responsibilities

  • Maintain complete, accurate, current clinical records for residents.
  • Ensure documentation is dated, signed, legible, and securely stored.
  • Manage admission, transfer, discharge, and death records.
  • Maintain master census and resident index.
  • Help ensure documentation is survey-ready.
  • Conduct routine audits of records, orders, charts, and assessments.
  • Track required documentation and reporting.
  • Identify deficiencies and coordinate resolutions with the team.
  • Support ongoing regulatory and HIPAA compliance.
  • Process Release of Information requests and coordinate with auditors.
