Job Overview

The LPN Medication Care Manager provides quality care by administering medication and treatments safely, adhering to laws, standards, and policies, to promote residents' quality of life.

Responsibilities

Medication Administration

• Administer medications and treatments safely and organize documentation.
• Review and document pertinent resident information.
• Follow the Six Rights of Medication Pass.
• Ensure timely medication passing and proper documentation.
• Maintain confidentiality and report concerns.
• Restock and check medications, maintain cleanliness, and assist with medication needs.

Resident Care

• Communicate resident and family concerns.
• Participate in team meetings and develop care plans.

Risk Management & Safety

• Ensure compliance with safety regulations and report hazards or incidents.
• Practice infection control and safe handling of hazardous materials.

Team Contribution & Training

• Participate as a team member, attend meetings, and complete required training.
• May supervise other staff and serve as shift supervisor.

Qualifications

• Licensed Practical Nurse (LPN, LVN) or equivalent credential.
• Maintain necessary certifications (CPR, First Aid).
• At least 18 years old, with preferred experience with seniors.
• Effective communication, judgment, and decision-making skills.
