Job Overview

The LPN Medication Care Manager is responsible for providing the highest quality care by administering medication and treatments safely and organized. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care in accordance with laws, standards, regulations, and Sunrise policies, all while promoting quality care and residents' quality of life.

Responsibilities

  1. Medication Administration:
    • Administer medications and treatments safely and organized.
    • Document medication administration accurately.
    • Follow the Six Rights of Medication Pass.
    • Maintain confidentiality and proper documentation.
    • Assist in medication checks and restocking.
    • Assess residents for 'as needed' medications and report accordingly.
    • Maintain cleanliness of medication areas.
    • Promote resident independence and dignity.
  2. Resident Care:
    • Communicate resident and family concerns.
    • Participate in care planning and updates.
    • Transcribe orders as needed.
  3. Risk Management & Safety:
    • Ensure compliance with safety and health regulations.
    • Report accidents and hazards promptly.
    • Follow infection control procedures.
    • Handle hazardous materials safely.
  4. Team Participation:
    • Engage actively with the team and support community programs.
    • Attend meetings and training sessions.
    • May serve as shift supervisor or supervise others.
Qualifications

Must have:

  • LPN, LVN, or equivalent licensed nurse credential.
  • Certifications in CPR and First Aid.
  • At least 18 years old.
  • Preferred experience with seniors and caring for the elderly.
  • Effective communication skills.
  • Good judgment, problem-solving, and decision-making skills.
  • Compliance with driving and safety policies if applicable.

Additional requirements include ongoing training and adherence to state/provincial regulations.
