Job Overview

The LPN Medication Care Manager is responsible for providing quality care by administering medication and treatments safely. Responsibilities include medication administration, documentation, resident care, and compliance with laws and policies to promote high-quality services.

Responsibilities & Qualifications
Medication Administration
  • Administer medications and treatments safely and accurately.
  • Document medication administration properly.
  • Follow the Six Rights of Medication Pass.
  • Maintain confidentiality of resident information.
  • Report concerns and assist in medication checks and restocking.
Resident Care
  • Communicate resident and family concerns.
  • Participate in care planning and updates.
Risk Management & Safety
  • Ensure compliance with safety and health regulations.
  • Report accidents and hazards immediately.
  • Practice infection control procedures.
Team Contribution & Training
  • Participate actively as a team member.
  • Attend meetings and training sessions.
  • May supervise other team members.
Qualifications
  • Licensed Practical Nurse (LPN) or equivalent.
  • Current certifications in CPR and First Aid.
  • At least 18 years old, with preferred experience with seniors.
  • Good judgment, communication skills, and ability to handle multiple priorities.
