Job Overview

The LPN Medication Care Manager provides quality care by administering medications and treatments safely. Responsibilities include medication administration, documentation, conducting SHUs, and resident care, all in accordance with laws, regulations, and Sunrise policies to ensure high-quality service.

Responsibilities:

• Medication Administration: Administer medications safely, document accurately, review medication updates, assist with and observe medication passes, ensure timely administration, maintain confidentiality, report concerns, restock supplies, and promote resident independence and dignity.
• Resident Care: Communicate resident and family concerns, participate in meetings, conduct service and health updates, develop and update care plans, and transcribe orders.
• Risk Management and Safety: Ensure compliance with safety regulations, practice safety procedures, report incidents, maintain equipment, and handle hazardous materials properly.
• Training and Team Contribution: Participate actively in team activities, attend meetings and training, possibly supervise others, and maintain compliance with training requirements.

Qualifications:

Must have a valid LPN, LVN, or state/province-specific nursing license, maintain certifications, be at least 18 years old, preferably have experience with seniors, and possess strong communication, judgment, and decision-making skills. Ability to react calmly in difficult situations and handle multiple priorities is essential. Driving a Sunrise vehicle requires signing the Driver job description and understanding safety duties.
