Job Overview

The LPN Medication Care Manager is responsible for providing the highest quality care by administering medication and treatments safely and organized. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care, all in accordance with laws, regulations, and Sunrise policies to ensure the highest quality of care.

Responsibilities & Qualifications
Responsibilities
  • Support our Mission, Principles of Service, and Core Values, emphasizing the sacred value of human life.
  • Administer medications and treatments, ensuring safety and proper documentation.
  • Review, read, notate, and document in the Daily Log about residents.
  • Receive medication updates from RCD or Wellness Nurse.
  • Follow the Six Rights of Medication Pass during administration.
  • Ensure medications are passed on time, and document refusals or missed doses.
  • Maintain confidentiality of resident information.
  • Report concerns and assist with medication restocking and checking.
  • Assess residents for as-needed medication needs and document appropriately.
  • Count narcotics with a colleague each shift.
  • Keep medication areas clean and organized.
  • Assist residents in maintaining independence and dignity.
  • Practice good hygiene and safety precautions.
  • Audit medication carts regularly.
Resident Care
  • Notify RCD of concerns via LEAD process.
  • Participate in daily meetings and update service plans.
  • Transcribe orders accurately.
Risk Management and Safety
  • Ensure compliance with safety regulations and report hazards or incidents immediately.
  • Practice infection control and handle hazardous materials properly.
  • Store oxygen tanks safely and monitor levels.
Training and Team Contribution
  • Participate actively in team activities and engagement programs.
  • Attend meetings and training sessions.
  • May serve as shift supervisor or supervise others.
  • Maintain required certifications and training.
Qualifications
  • Licensed as LPN, LVN, or equivalent.
  • Maintain certifications like CPR and First Aid.
  • At least 18 years old.
  • Experience with seniors preferred.
  • Strong communication, judgment, and decision-making skills.
