Job Overview

The LPN Medication Care Manager is responsible for providing high-quality care by administering medication and treatments safely and organized. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care, all in accordance with relevant laws, standards, regulations, and Sunrise policies, to promote the highest quality of care.

Responsibilities & Qualifications

Need Sundays - Thursdays 3P-11P, Full Time

Responsibilities:

• Administer medications and treatments safely and organized.
• Document medication administration accurately.
• Conduct SHUs and participate in resident care.
• Support the development and updates of Individualized Service Plans.
• Participate in safety and risk management practices, including reporting incidents and hazards.
• Support team success through participation in meetings, training, and engagement programs.

Qualifications:

• Licensed Practical Nurse (LPN), LVN, or equivalent license.
• Maintain required certifications (CPR, First Aid, etc.).
• Minimum age 18 years.
• Previous experience with seniors preferred.
• Strong communication, judgment, and decision-making skills.
