Job Overview

The LPN Medication Care Manager is responsible for providing the highest quality care by administering medication and treatments safely and organized. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care in accordance with laws, standards, and policies, to promote quality of life for our residents.

Responsibilities & Qualifications

Schedule: 3-11PM, Tuesday through Saturday

Responsibilities

• Provide quality care by administering medications and treatments safely.
• Document medication administration accurately.
• Review and update resident logs and medication records.
• Assist with medication updates and ensure proper storage and re-fill processes.
• Maintain confidentiality and report concerns appropriately.
• Support resident independence and dignity, adhering to Sunrise Principles of Service.
• Participate in team meetings and contribute to resident care planning.
• Follow safety and infection control protocols.
• Assist in medication audits and restocking.

Qualifications

• Licensed Practical Nurse (LPN) or equivalent credential.
• CPR and First Aid certifications (must be maintained).
• Previous experience with seniors preferred.
• Strong communication, judgment, and decision-making skills.
• Ability to react calmly in difficult situations.
• Must be at least 18 years old.
