Qualifications

• Graduation from an accredited school of Nursing.
• Licensed Practical Nurse (LPN) with MBON active status.
• Current AHA BLS (HCP) certification.

Responsibilities

• Meets with residents to collect RAI data and assess health status.
• Recommends nursing interventions based on assessments and team input.
• Completes focused resident assessments (initial, quarterly, annual).
• Completes MDS/MMDS per policy and regulatory requirements.
• Contributes to care plan development and reviews progress.
• Educates care-delivery staff on MDS procedures and documentation.
