Job summary

Keswick, located in Baltimore, MD, seeks an RNAC/MDS Coordinator to implement and evaluate the MDS program and support care planning in alignment with Keswick’s nursing philosophy.

The role requires an RN license, BLS, and 3+ years as an MDS Coordinator/RNAC, with RAC-CT to be obtained within 6 months. This is a full-time, day-shift opportunity in a not-for-profit aging services setting.

Qualifications

• RN license in good standing with MBON.
• Current AHA BLS certification.
• Three years of experience as an MDS Coordinator/RNAC.
• RAC-CT must be obtained within 6 months of hire.

Responsibilities

• Meet with residents to collect information for RAI and assess health status.
• Develop nursing diagnoses and care plans; recommend interventions.
• Communicate with health team members and unit management.
• Ensure complete documentation to substantiate reimbursement.
• Complete MDS/MMDS and transmit to the State.
• Educate care-delivery staff on MDS processes.
• Monitor regulatory compliance and participate in surveys.
