Job summary

Alumus is seeking a Licensed Practical Nurse (LPN/LVN) to provide in-home nursing care, utilizing the nursing process to deliver high-quality, continuous care at the patient’s residence.

The role requires current licensure, minimum 2 years of LPN/LVN experience, and the ability to educate patients and families on disease processes, medications, and wound care. Join a compassionate team focused on patient advocacy and holistic care.

Qualifications

• Current, unencumbered LPN/LVN license required or Compact State license.
• A minimum of 2 years LPN/LVN experience required.
• Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.

Responsibilities

• Complete and scan required record keeping forms/charts upon admission, transfer, and discharge.
• Chart nurses’ notes reflect care provided and patient response in the medical record.
• Report discrepancies to the Director of Nursing.
• Ensure documentation of unauthorized discharges is entered in the guest’s medical record per procedures.
