Alumus is seeking a Licensed Practical Nurse (LPN/LVN) to provide in-home nursing care, following the nursing process for optimal quality and continuity of care in patients’ residences.

Responsibilities include charting, documenting patient response, and reporting discrepancies to the Director of Nursing. Must have a current LPN/LVN license and at least 2 years of experience, with ability to educate patients and families on disease processes and medications.

Competenze

• Current, unencumbered LPN/LVN license required or Compact State license.
• A minimum of 2 years LPN/LVN experience required.
• Ability to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.

Mansioni

• Complete and scan required record keeping forms/charts upon the guest’s admission, transfer, and/or discharge.
• Chart nurses’ notes reflecting care provided and patient response.
• Report discrepancies concerning physician’s orders, diet changes, charting errors to the Director of Nursing.
• Ensure documentation for unauthorized discharges is entered in the patient’s medical record.
