Job summary

Alumus is seeking a Licensed Practical Nurse (LPN) to provide in-home nursing care, utilizing the nursing process to ensure optimum quality and continuity of care for patients in their residence.

The role requires an active LPN/LVN license (or Compact state), at least 2 years of experience, and the ability to educate patients and families on disease processes and wound care. Join a team that values lifelong learning, supportive supervision and flexible scheduling.

Qualifications

• Current, unencumbered LPN/LVN license required (or Compact State license).
• Minimum of 2 years LPN/LVN experience required.
• Ability to educate patients and family on multiple disease processes and wound care.

Responsibilities

• Complete and scan required record keeping forms/charts upon the guest’s admission, transfer, and/or discharge.
• Chart nurses’ notes reflecting care provided and patient response.
• Report discrepancies to the Director of Nursing.
• Ensure documentation of unauthorized discharges is entered in the medical record.
