Job summary

Sunrise Senior Living in Charlottesville, VA is seeking a licensed Practical Nurse (LPN) to support resident health and wellbeing. You’ll perform assessments, monitor medications, maintain MAR/TAR accuracy, and collaborate with the care team to ensure high-quality care.

Responsibilities

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments
• Perform monthly wellness visits and monitor health trends
• Follow up on falls, incidents, and hospital returns
• Transcribe physician orders and ensure accurate MAR/TAR documentation
• Complete medication audits and oversee medication compliance
• Partner with families, physicians, and the Resident Care Director
• Support infection control and regulatory standards
• Educate and guide care staff on documentation and medication practices

What We’re Looking For:
