Alumus is seeking a Licensed Practical Nurse (LPN/LVN) to provide in-home nursing care, using the nursing process to ensure quality and continuity of care for residents in their homes.

Qualifications

• Current, unencumbered LPN/LVN license required; compact state license acceptable.
• Minimum 2 years LPN/LVN experience.
• Ability to educate patients and families on disease processes and wound care.

Responsibilities

• Complete and scan required record-keeping forms upon admission, transfer, and/or discharge.
• Chart nurses’ notes reflecting care provided and patient response.
• Report discrepancies concerning physician orders, diet changes, or charting errors to the Director of Nursing.
• Ensure documentation of unauthorized discharges per established procedures.
