Job summary

Alumus Healthcare is seeking a Licensed Practical Nurse (LPN/LVN) to provide in-home nursing care, following the nursing process to ensure quality and continuity of care for patients in the comfort of their residence. The role requires a valid LPN/LVN license and at least 2 years of hands-on experience.

Responsibilities include charting nursing notes, reporting order discrepancies, and documenting any unauthorized discharges.

Qualifications

• Must hold an active LPN/LVN license (or compact state license).
• Minimum of 2 years of LPN/LVN experience required.
• Ability to educate patients and families on multiple disease processes and wound care.

Responsibilities

• Complete and scan required record keeping forms/charts upon admission, transfer, or discharge.
• Chart nurses' notes reflecting care provided and patient response.
• Report discrepancies to the Director of Nursing.
• Ensure documentation of unauthorized discharges per procedures.
