Job summary

Amedisys is seeking a licensed practical nurse to provide in-home care as part of our dedicated home health team. You will verify plans of care with RNs, collaborate with physicians, and educate patients and families on self-care. Expect routine vital checks, medication review, and aseptic preparation for treatments.

The role emphasizes compassionate, patient-centered care with everyday opportunities to improve quality of life in the patient's home, with full‑time daytime hours.

Qualifications

• Current LPN license, specific to the state(s) you are assigned to work.
• One year of clinical LPN experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.

• $29 - $37 Base Rate (Converting to Per Visit)
