Job summary

Amedisys Home Health is seeking a dedicated Licensed Practical Nurse for full-time days. Join one of the largest home health providers delivering care in patients' homes with flexibility and growth opportunities.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) assigned.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs during visits.
• Review medications and perform medication reconciliation at each visit.
• Perform physician-ordered procedures (e.g., Foley catheter changes).
• Prepare equipment and materials using aseptic technique.
• Complete documentation timely and accurately.
