Job summary

Amedisys Home Health in the United States is seeking a licensed practical nurse to join our full-time days team. You will verify plans of care, collaborate with RNs and physicians, and provide patient education in the home setting. A valid driver’s license, reliable transportation and CPR certification are required. Travel to patient homes is required; mileage is reimbursed.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state-specific.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g., Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
