Job summary

Amedisys Home Health is seeking a licensed practical nurse for full-time day shifts in home health setting. You will provide one-to-one patient care, verify plans of care with the RN, and collaborate with physicians to ensure patient needs are met at home.

Compensation ranges from $25 to $34 per hour with benefits, including medical plans, PTO, retirement options and wellness rewards. Eligibility varies by position and shift.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state-specific.
• Current CPR certification.
• Valid driver's license and transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs and perform medication reconciliation.
• Prepare equipment for treatments using aseptic technique.
• Complete documentation timely and accurately.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
