Job summary

Amedisys is seeking a licensed practical nurse for full-time day shifts in Atlanta, GA. You will verify plans of care with RNs, collaborate with physicians, assess patients, and provide self-care education to families in the home setting.

Responsibilities include vitals, medication reconciliation, sterile prep, and accurate documentation. Must hold an active LPN license in GA and CPR certification, with reliable transportation and a valid driver’s license.

Qualifications

• Active LPN license in the assigned state(s).
• Minimum of 1 year clinical LPN experience.
• CPR certification current and in good standing.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and report changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs at visits and reconcile medications.
• Prepare equipment for treatments using aseptic technique.
• Complete documentation timely and accurately.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
