Job summary

Amedisys is seeking a licensed practical nurse for a full-time Day shift in Lancaster, SC. This home health role involves visiting patients at home, verifying care plans with RNs, and coordinating with physicians to ensure patient needs are met.

Mileage reimbursement and comprehensive benefits are included. The ideal candidate has an active LPN license, 1 year of clinical LPN experience, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
